Site Checklist

Introduction to practice manager
- Went into the practice on the Thursday to introduce ourselves to Sarah Dunn and discuss the scope of work and time frame but Sarah was on leave so spoke to the girl on reception
Address any practice manager concerns
- There weren’t any specific concerns
Inspect exterior perimeter of practice where applicable
Inspect signage for damage
- Five car park signs are showing weather deterioration and the main Queensland X Ray sign above the front main door has lost its colour
Inspect any allocated car parks
Check for fire system that may require isolation
Check floor coverings for damage
Check all doors for correct operation
Check all doorstops are functional
Inspect walls for damage
- We found it very badly worn with lots of marks , holes and chipped corners where there is no corner protection
Check over joinery units for damage
- Found one cupboard door in one of the ultra sound rooms was loose due to screws falling out, refitted using larger screws
Check inside each cabinet for water leaks
Check splash backs for water damage
Check all silicone to splash backs, basins, sinks
Check soap and hand towel dispenser condition
Check for damaged ceiling tiles
- Found water stained tiles and old holes where things have been removed but did not have time to replace any tiles in this scheduled maintenance
Check all lights are functioning
- We found 2 of the dimmer knobs in the centre control room missing , trying to source the right sized knobs

Most of allocated time was spent plastering and painting on this practice as it was very worn. Didn’t replace water stained or damaged ceiling tiles as they weren’t that bad possibly nine tiles could be replaced next time in maintenance also would recommend wall protection in backdoor entrance linen room where trolleys are knocking gyprock also wall protection behind all rubbish bins as they are damaging the paint sometimes on the same day it was repaired and painted		Actioned
